Yes. Home is the most common care setting. Our nurses, aides, social workers and chaplains visit the patient at their residence. Medical equipment and medications related to the hospice diagnosis are coordinated and delivered as needed.
Same-day evaluation may be available when clinically appropriate and operationally available. After a referral or intake call, a nurse will schedule an in-person visit to complete the clinical assessment and enrollment paperwork.
Hospice is appropriate when a physician certifies that a patient has a terminal illness with a prognosis of six months or less if the illness follows its natural course, and the patient and family choose comfort-focused care over curative treatment.
Yes. Our team coordinates with hospital social workers and discharge planners at Community Memorial Hospital and other area facilities to ensure smooth transitions from hospital to home-based hospice care.
