Eternal Life Hospice Inc. ("Eternal Life Hospice," "we," "our," or "us") is a Medicare-certified, CDPH-licensed, and ACHC-accredited hospice care provider serving families across Ventura County and Los Angeles County, California. Our principal office is located at:

This Privacy Policy applies to the website located at eternallifehospice.com and any subpages (the "Site"). It does not apply to information collected through other channels, such as phone calls, in-person admissions, or clinical care records, which are governed separately by our HIPAA Notice of Privacy Practices.

2. Information We Collect

We collect two categories of information through the Site:

Information you give us directly

  • Contact and referral forms — name, phone number, email address, relationship to patient, and any message you choose to include when you submit a "Contact Us," "Refer a Patient," or similar form.
  • Newsletter sign-ups — email address and first name when you subscribe to the Eternal Care Brief.
  • Job applications — name, contact details, résumé, and role interest submitted through the Careers page.
  • Volunteer inquiries — name, contact details, and availability submitted through the Volunteer page.

Information collected automatically

  • Browser type, operating system, device type, and screen resolution.
  • Pages visited, time on page, scroll depth, and referring URL.
  • General geographic location (city/region level — derived from IP address, not GPS).
  • Mouse movements, clicks, and session recordings (see Section 4 — Microsoft Clarity).
  • IP address (used only for analytics aggregation; not stored in identifiable form by us).

We do not collect Social Security numbers, financial account numbers, medical record information, or protected health information (PHI) through this website.

3. How We Use Your Information

We use the information we collect to:

  • Respond to your inquiry, referral, or application.
  • Send the Eternal Care Brief newsletter to subscribers who opted in.
  • Understand how visitors use the Site so we can improve it (analytics).
  • Detect and prevent fraudulent or abusive traffic on paid advertising campaigns.
  • Comply with legal obligations and defend our legal rights.

We do not sell, rent, or trade your personal information to any third party for their own marketing purposes.

All analytics scripts are loaded after the page's initial render to avoid any impact on page load performance.

5. Cookies & Tracking

Cookies are small text files placed on your device by your browser. This Site uses:

  • Essential cookies — required for basic site functionality (e.g., form security tokens). Cannot be disabled without breaking the site.
  • Analytics cookies — set by Google Analytics and Microsoft Clarity to distinguish unique visitors and sessions. These are first-party cookies stored in your browser.
  • Marketing/tracking cookies — set by Brevo when you are a newsletter subscriber, to understand which pages you visit before and after opening an email.

You can update your cookie preferences at any time using the Cookie Settings link in the footer of every page on this Site. You can also opt out of Google Analytics tracking using the Google Analytics Opt-out Browser Add-on, or opt out of Microsoft Clarity at clarity.microsoft.com/optout. Most browsers also allow you to block or delete cookies through their settings menus.

6. A Note on HIPAA

This website is not a HIPAA-covered channel for clinical information. Do not submit protected health information (PHI) — diagnosis details, medical record numbers, insurance IDs, or clinical histories — through any form on this Site. Please call us directly at 805.953.7273 to discuss a patient's clinical situation.

Eternal Life Hospice is a HIPAA-covered entity in our capacity as a hospice care provider. Our HIPAA Notice of Privacy Practices governs how we handle patient health information collected through clinical intake, care delivery, and billing processes — not through this website. If you have questions about your rights as a patient under HIPAA, please contact our Privacy Officer at info@eternallifehospice.com.

7. California Privacy Rights (CCPA / CPRA)

If you are a California resident, the California Consumer Privacy Act (CCPA) as amended by the California Privacy Rights Act (CPRA) gives you the following rights with respect to personal information we hold about you:

  • Right to Know — you may request a copy of the categories and specific pieces of personal information we have collected about you in the past 12 months.
  • Right to Delete — you may request that we delete personal information we have collected from you, subject to certain legal exceptions.
  • Right to Correct — you may request that we correct inaccurate personal information we hold about you.
  • Right to Opt Out of Sale or Sharing — we do not sell or share personal information for cross-context behavioral advertising. No opt-out is required, but you may contact us to confirm this at any time.
  • Right to Non-Discrimination — we will not deny services, charge different prices, or provide a different level of quality because you exercised any of the above rights.

To submit a rights request, please email info@eternallifehospice.com with the subject line "California Privacy Request" or call us at 805.953.7273. We will verify your identity and respond within 45 days as required by law.

We do not knowingly sell personal information. In the 12 months preceding the effective date of this policy, Eternal Life Hospice has not sold any personal information collected through this Site.

8. Data Retention

We retain personal information only as long as necessary for the purpose for which it was collected, or as required by law:

  • Form submissions (contact, referral, volunteer) — retained for up to 3 years, then deleted unless a business relationship resulted.
  • Job applications — retained for 2 years after the position is filled or the application is closed.
  • Newsletter subscriber data — retained until you unsubscribe, after which it is removed from active lists within 30 days.
  • Analytics data — Google Analytics data is retained for 14 months (our configured retention setting). Microsoft Clarity session recordings are retained for 90 days per Clarity's default policy.

9. Children's Privacy

This Site is not directed at children under the age of 13. We do not knowingly collect personal information from children under 13. If you believe a child has submitted personal information through this Site, please contact us at info@eternallifehospice.com and we will promptly delete it.

The Site contains links to external websites — including Medicare.gov, CDPH, ACHC, and other healthcare and government resources. We are not responsible for the privacy practices of those sites. We encourage you to review the privacy policies of any site you visit through a link on our pages.

11. Changes to This Policy

We may update this Privacy Policy from time to time to reflect changes in our practices, technology, or legal requirements. When we do, we will revise the effective date at the top of this page. If changes are material, we will provide a more prominent notice. Your continued use of the Site after any update constitutes your acceptance of the revised policy.
