3. No Medical Advice

Nothing on this Site should be used to make medical decisions about hospice eligibility, patient care, medication management, or any other clinical matter. Always consult a qualified healthcare professional for guidance specific to your or your loved one's situation.

Submitting a form on this Site does not establish a patient-provider relationship between you and Eternal Life Hospice. A formal care relationship is established only through a signed service agreement and physician order.

All content on this Site — including but not limited to text, graphics, logos, photographs, audio recordings, video clips, icons, and software — is the property of Eternal Life Hospice Inc. or its content suppliers and is protected by United States and international copyright, trademark, and other intellectual property laws.

By submitting information, you grant Eternal Life Hospice a non-exclusive, royalty-free license to use that information for the purpose for which it was submitted (e.g., responding to your inquiry, processing your referral, contacting you about your application).

Please do not submit protected health information (PHI), including patient diagnoses, medical record numbers, insurance IDs, or clinical histories, through any form on this Site. For clinical matters, please call us directly at 805.953.7273.

Any dispute arising out of or relating to these Terms or your use of the Site shall be resolved exclusively in the state or federal courts located in Ventura County, California. By using the Site, you consent to the personal jurisdiction of those courts and waive any objection to that venue.

You and Eternal Life Hospice agree that any cause of action arising out of or related to this Site must be commenced within one (1) year after the cause of action accrues. Otherwise, such cause of action is permanently barred.

10. Changes to These Terms

We reserve the right to modify these Terms at any time. When we do, we will update the effective date at the top of this page. If changes are material, we will make reasonable efforts to provide notice, such as a prominent notice on the Site.

Your continued use of the Site after any change to these Terms constitutes your acceptance of the revised Terms. If you do not agree to the revised Terms, you must stop using the Site.
